Our office policy allows us to schedule your initial appointment as quickly as possible. Please alert our staff if this is an accident or your pain intensity is high, we will strive to make you an emergency appointment immediately. As in all offices you will be asked to first fill out a few forms. Some forms are required to help us better understand your condition and other forms are required by governmental regulations and third party payers.
